Cats express anxiety very differently from dogs -- they tend to hide, withdraw, over-groom, or develop physical symptoms like urinary blockages rather than vocalizing or acting out. Chronic stress in cats is linked to idiopathic cystitis (bladder inflammation), upper respiratory flares, and immune suppression. Identifying and addressing the source of anxiety -- before it escalates to medical problems -- is the most effective approach.

Common Stressors in Domestic Cats

Cats are territorial and sensitive to change. The most common stressors: new animals in the home (even if behind closed doors initially); changes in routine (owner's schedule changing, new work-from-home arrangements that later end); outdoor cats visible through windows (a major stressor many owners don't consider); inadequate resources in multi-cat homes (not enough litter boxes, food stations, or vertical space); loud or unpredictable noise; and veterinary visits.

Pheromone products (Feliway Classic diffuser) release synthetic F3 facial pheromone, which cats deposit when rubbing their faces on objects -- a self-calming behavior. Clinical studies show meaningful reduction in stress-related behaviors compared to placebo. It takes 2-4 weeks to see the full effect. For acute stressors like vet visits, gabapentin given 1.5-2 hours before the appointment dramatically reduces fear responses in most cats -- ask your vet about this option.

When to Add Medication to the Plan

Environmental modification and pheromone therapy are first-line -- but for cats with recurrent FIC, psychogenic alopecia, multi-cat aggression, or severe separation anxiety, medication significantly accelerates progress. Fluoxetine (daily SSRI) is commonly used for chronic anxiety and has the best evidence base for cats. Gabapentin works for situational anxiety. Buspirone is sometimes used for inter-cat aggression. Medication works best in combination with behavioral modification and environmental enrichment, not as a standalone fix.
