What do these old terms mean, and why do vets now say disorder of sexual development?

The old terms describe how gonads and genital appearance line up, but they can sound confusing and outdated to owners. Merck still defines true hermaphrodites as animals with both ovarian and testicular tissue and describes pseudohermaphroditism as a condition in which gonadal sex and phenotypic sex do not match. Current literature increasingly uses DSD because it is broader, more accurate, and less stigmatizing when multiple chromosomal, gonadal, and anatomic variations are possible (Merck Veterinary Manual, 2026).

That wording shift matters because owners often come in expecting a simple male-or-female answer, while the real question is how chromosomes, gonads, internal ducts, and external genitalia developed in that individual cat. A cat may have testes with female-appearing external structures, ovotesticular tissue, or a chromosomal pattern that does not behave the way people expect. The newer DSD language helps veterinarians explain the condition as a developmental variation that needs mapping, not as a curiosity to be labeled quickly (Meyers-Wallen et al., 2012; Nowacka-Woszuk et al., 2022).

How might a cat with a sexual-development disorder first come to attention?

Some cats are brought in because of visibly ambiguous genitalia, such as an enlarged clitoris, a vulva-like opening paired with a penile structure, absent scrotal testes, or unusual perineal anatomy. Others are discovered later when a supposedly female cat shows male-like behavior, when neutering surgery does not match expectations, or when a retained gonad is found in the abdomen. Merck notes that male pseudohermaphrodites may have testicular tissue in the abdomen or beneath the skin with external genitalia resembling those of females (Merck Veterinary Manual, 2026).

The clue can also be surprisingly indirect. The 17-cat cytogenetic series included cats investigated for abnormal external genitalia, unusual behavior, or even tricolor coat patterns atypical in males. That reminder is useful because DSDs are not always obvious at kittenhood. Sometimes the problem surfaces only when puberty, marking behavior, infertility, recurrent urinary issues, or an unexpected surgical finding raises the question of whether the cat's reproductive anatomy developed along more than one pathway (Nowacka-Woszuk et al., 2022).

What actually causes these conditions in cats?

The causes can include chromosomal abnormalities, sex-chromosome mosaicism, gene translocations, or disruption in the signaling that guides gonadal and genital development. Merck explains the basic developmental rule: XY genotype normally leads to testes because of the SRY gene, and those testes then drive further masculine development. When that sequence is interrupted, misplaced, duplicated, or mixed with other chromosomal changes, the final anatomy may not match the expected pattern (Merck Veterinary Manual, 2026).

Cat studies show how variable that can be in practice. Meyers-Wallen and colleagues described an SRY-positive 38,XY cat with ovotestes, while the broader 17-cat analysis documented several chromosome complements and gonadal outcomes across affected cats. In other words, one external appearance can arise from several different genetic stories, which is why diagnosis based on looks alone is so unreliable (Meyers-Wallen et al., 2012; Nowacka-Woszuk et al., 2022).

Why is a precise diagnosis more complicated than just looking at the genitals?

External anatomy is only one layer of sex development. A veterinarian may need to know what gonadal tissue is present, whether uterine horns or deferent ducts exist, whether testes are descended, and what chromosome pattern the cat carries. Merck describes male pseudohermaphrodites as potentially having uterine horns, a cervix, and portions of the vagina along with testicular tissue, which shows how internal and external structures can tell very different stories (Merck Veterinary Manual, 2026).

The published feline cases make that point even more strongly. The SRY-positive XY true hermaphrodite report used gross anatomy, histology, and molecular testing to reach a final diagnosis, and the later series relied on cytogenetic, molecular, and histologic analysis across multiple cats. Owners sometimes hope one ultrasound will settle everything, but definitive classification often requires several diagnostic tools working together (Meyers-Wallen et al., 2012; Nowacka-Woszuk et al., 2022).

What tests do veterinarians usually recommend for these cats?

The workup often includes physical examination, abdominal imaging, hormone-aware interpretation of behavior or reproductive signs, and surgical exploration or biopsy of gonadal tissue when needed. Imaging can look for retained testes, uterine structures, or associated urinary abnormalities, while histopathology tells the team whether tissue is ovarian, testicular, ovotesticular, or dysgenetic. Because cryptorchid tissue carries health implications of its own, locating any retained gonad matters even before the full genetic story is complete (Merck Veterinary Manual, 2026).

Karyotyping and molecular tests add another layer when available. The 17-cat study demonstrates how chromosome analysis can uncover mosaicism, XX/XY patterns, or other abnormalities that would be invisible on routine exam. For owners, the practical point is that a DSD diagnosis often takes longer than a standard neuter consult because the team is solving an anatomy-and-genetics puzzle, not merely planning a simple reproductive surgery (Nowacka-Woszuk et al., 2022).

What signs in a cat with ambiguous genitalia make prompt veterinary evaluation more important?

Prompt evaluation is important if the cat strains to urinate, has recurrent urinary tract signs, develops swelling or pain around the genital opening, shows discharge, or has a palpable abdominal mass. Those problems may reflect retained gonadal tissue, urinary obstruction, infection, or anatomy that needs more than cosmetic explanation (Merck Veterinary Manual, 2026).

It also matters when a planned spay or neuter reveals unexpected structures. That is not a reason to panic, but it is a reason to slow down and let the team document what they are seeing carefully. Ambiguous anatomy paired with illness or surgical surprise should trigger a structured diagnostic workup rather than assumptions based on appearance (Meyers-Wallen et al., 2012; Nowacka-Woszuk et al., 2022).

Bottom line

Cats described as hermaphrodites or pseudohermaphrodites usually have a disorder of sexual development that cannot be understood from external appearance alone. The safest path is careful imaging, tissue diagnosis, and, when available, chromosome or molecular testing before final decisions are made. That slower approach helps prevent incomplete surgery, mistaken assumptions, and long-term confusion about what tissue is actually present.

Does every cat with a DSD need surgery or treatment?

Treatment depends on symptoms, anatomy, retained gonads, and future health risks rather than on appearance alone. A retained abdominal testis may justify removal because cryptorchid gonads can twist, cause hormone-driven behavior, or complicate later disease risk. Surgery may also be recommended if abnormal anatomy is causing recurrent urinary infections, discharge, trauma, or owner confusion about heat-like behavior (Merck Veterinary Manual, 2026).

Not every case is an emergency, though. Some cats are otherwise healthy and come to attention only because of unexpected anatomy discovered during sterilization planning. In those situations, the best care may be deliberate diagnosis first and surgery second, so the veterinarian can remove the correct tissues and explain the findings accurately. Rushing to simplify the anatomy without understanding it can leave tissue behind or miss clinically important structures (Meyers-Wallen et al., 2012; Nowacka-Woszuk et al., 2022).

What should owners expect long term once the diagnosis is confirmed?

Long-term outlook is often good when the cat's urinary tract is functional, retained gonads are addressed appropriately, and no major concurrent defects are present. The biggest challenges tend to be diagnostic uncertainty, behavior related to hormone production, and the occasional need for staged decision-making when internal anatomy is unusual. Once those issues are mapped and managed, many cats can live comfortable lives without ongoing instability (Merck Veterinary Manual, 2026).

What owners need most is clear communication. These cases can become emotionally loaded because the terminology is unfamiliar and internet advice is chaotic. A good veterinary team explains what tissue was found, what was removed or retained, what the chromosome information means if tested, and whether any follow-up is needed. The diagnosis becomes much less frightening when it is translated into concrete anatomy, concrete risks, and a concrete plan (Nowacka-Woszuk et al., 2022; Meyers-Wallen et al., 2012).
