Hyphema in Cats: When Blood in the Eye Signals a Whole-Body Emergency

Hyphema is the presence of blood within the anterior chamber of the eye β€” the fluid-filled space between the cornea and the iris. It is never a primary disease; it is always a sign that something has broken the blood-ocular barrier, and that something can range from a blunt-force injury to a systemic bleeding disorder to an aggressive cancer. The amount of blood can vary from a faint pinkish haze suspended in the aqueous humor to a settled pool of bright red cells at the bottom of the chamber, to a completely blood-filled anterior chamber that renders the eye blind and visibly dark (Merck Veterinary Manual, 2024).

The blood usually originates from the iris blood vessels, but it can also come from the ciliary body behind the iris, the choroidal vessels beneath the retina, or the retinal vessels themselves. When the bleeding is extensive or recurrent, the entire anterior chamber can fill with clotted blood β€” a condition called total hyphema or "eight-ball hemorrhage" β€” which carries a high risk of secondary glaucoma because the clot physically blocks the drainage angle where aqueous humor normally exits the eye (CriticalCareDVM, n.d.).

What causes blood to pool inside a cat's eye?

The causes of hyphema fall into several broad categories. Trauma β€” blunt or penetrating β€” is the most immediately obvious. A cat hit by a car, kicked by a horse, or struck by a swinging door may develop hyphema within hours, often accompanied by conjunctival hemorrhage, eyelid swelling, or orbital fractures. But spontaneous hyphema, where no trauma history exists, is more diagnostically challenging and often more serious (Merck Veterinary Manual, 2024).

Systemic hypertension is a common cause in older cats. When arterial blood pressure climbs above 160-180 mmHg, the fragile vessels of the iris and retina can rupture, producing hyphema, retinal detachment, or both. Chronic or severe uveitis can erode blood vessels and cause bleeding into the anterior chamber. Coagulation disorders β€” from rodenticide toxicity, inherited clotting factor deficiencies, severe liver disease, or thrombocytopenia β€” prevent normal clot formation. Infectious diseases such as FIP, FeLV, and FIV can cause uveitis and vasculitis that lead to hyphema. Intraocular neoplasia β€” lymphoma, melanoma, or metastatic tumors β€” can invade and erode ocular blood vessels (CriticalCareDVM, n.d.).

How do veterinarians figure out what is causing the bleeding?

A complete ophthalmic examination is the starting point. The veterinarian assesses the extent and appearance of the blood, checks for corneal injury or lens luxation, measures intraocular pressure with tonometry (elevated pressure suggests glaucoma or an intraocular tumor; low pressure suggests uveitis), and examines the fundus (retina and optic nerve) if the hyphema is not total. A Schirmer tear test and fluorescein stain rule out corneal ulceration. The presence or absence of pain β€” squinting, tearing, photophobia β€” helps distinguish uveitis from glaucoma (Merck Veterinary Manual, 2024).

Systemic diagnostics are equally important. Blood pressure measurement is mandatory in any cat with spontaneous hyphema. A complete blood count, serum biochemistry panel, coagulation profile (PT, PTT), and platelet count screen for systemic disease. Testing for FeLV, FIV, and FIP may be indicated. Thoracic and abdominal imaging β€” radiographs and ultrasound β€” can identify primary tumors or evidence of trauma. Ocular ultrasound is useful when the anterior chamber is too opaque for fundic examination (CriticalCareDVM, n.d.).

What treatments can save the eye and the cat's vision?

Treatment has two goals: manage the eye itself and treat the underlying cause. Topical corticosteroids (prednisolone acetate or dexamethasone) reduce intraocular inflammation and stabilize the blood-ocular barrier. Topical atropine dilates the pupil, which prevents the iris from adhering to the lens (posterior synechia), reduces pain from ciliary muscle spasm, and may help redistribute blood away from the drainage angle. However, atropine is contraindicated if glaucoma is present, because pupil dilation can further compromise aqueous outflow (Merck Veterinary Manual, 2024).

Systemic treatment depends on the cause. Hypertensive cats need antihypertensive therapy β€” amlodipine is the first-line choice, with a target systolic blood pressure below 160 mmHg. Cats with coagulopathies may require vitamin K1 (for rodenticide toxicity), fresh frozen plasma, or platelet transfusions. Uveitis from infectious disease requires specific antimicrobial or antiviral therapy. Intraocular tumors may necessitate enucleation (removal of the eye) if the tumor is malignant and the eye is blind and painful. Cats with total hyphema and secondary glaucoma that cannot be controlled medically may also require enucleation for pain relief (CriticalCareDVM, n.d.).

What determines whether a cat keeps its vision?

Prognosis for vision depends on four factors: the severity of the hyphema (mild versus total), whether the pupil can be dilated during treatment, whether there is concurrent damage to the retina or optic nerve, and how quickly the underlying cause is controlled. Mild hyphema that resolves with topical therapy and treatment of the underlying disease carries a good prognosis for return of functional vision. Total hyphema that fails to clear, or hyphema complicated by retinal detachment or uncontrolled glaucoma, often results in permanent blindness in the affected eye (Merck Veterinary Manual, 2024).

Even when vision is lost in one eye, cats adapt remarkably well. A one-eyed cat can navigate, hunt, and live a full life. The greater concern is whether the underlying disease β€” hypertension, cancer, coagulopathy β€” threatens the other eye or the cat's overall survival. Regular follow-up with intraocular pressure checks and blood pressure monitoring is essential for cats with a history of hyphema, because recurrence is common if the underlying condition is not fully controlled.

When to worry

A suddenly enlarged, hard, or protruding eye suggests acute glaucoma β€” emergency care is needed. Pawing at the eye, vocalizing, or keeping the eye tightly shut signals significant pain. Sudden blindness in both eyes, disorientation, or walking into walls suggests bilateral retinal detachment or severe hypertension. Hyphema with systemic signs β€” collapse, pale gums, bruising elsewhere, or bleeding from nose or mouth β€” needs immediate emergency evaluation for possible coagulopathy (CriticalCareDVM, n.d.).

Hyphema is blood where blood does not belong, and it always means something is wrong β€” in the eye, in the body, or both. The diagnostic workup must look beyond the eye to blood pressure, clotting function, infectious disease, and cancer. With prompt diagnosis and appropriate therapy, many cats retain functional vision. The key is acting quickly: the longer blood sits in the anterior chamber, the higher the risk of permanent damage.

Hyphema, or blood in the anterior chamber of the eye, is always a significant finding that warrants thorough investigation. The blood may appear as a settled red line at the bottom of the anterior chamber, a diffuse reddish haze, or a completely blood-filled chamber that obscures the iris and pupil. Systemic hypertension is a particularly important cause in older cats, often secondary to chronic kidney disease or hyperthyroidism, and may present with acute blindness due to retinal detachment in addition to hyphema. Because the eye is one of the few places where blood vessels can be directly visualized, ocular examination provides a window into systemic vascular health (Merck Veterinary Manual, 2025; Cornell University College of Veterinary Medicine, 2021). The diagnostic approach includes a complete ophthalmic examination, blood pressure measurement, complete blood count, serum biochemistry, coagulation profile, and ocular ultrasound when the anterior chamber is too opaque for direct visualization. Treatment is directed at the underlying cause while also managing the ocular consequences. Topical corticosteroids may be used to control uveitis, and topical atropine can help stabilize the blood-aqueous barrier and reduce discomfort from ciliary spasm. Intraocular pressure must be monitored closely because hyphema can obstruct the iridocorneal angle and lead to secondary glaucoma. In severe cases where the blood does not resorb, surgical intervention may be necessary to prevent permanent corneal blood staining and vision loss (Merck Veterinary Manual, 2025).

The prognosis for hyphema depends entirely on the underlying cause and the timeliness of treatment. Traumatic hyphema in an otherwise healthy cat may resolve completely with appropriate medical management, though secondary glaucoma and synechiae formation remain potential complications. Hyphema secondary to systemic hypertension carries a better prognosis when the hypertension is identified and controlled early, before permanent retinal or renal damage occurs. When hyphema is caused by intraocular neoplasia, the prognosis is guarded and enucleation may ultimately be necessary for pain control. Regardless of cause, cats with hyphema require regular ophthalmic rechecks to monitor intraocular pressure, assess clot resolution, and detect complications. Owners should understand that even after the visible blood has cleared, the underlying disease process may require ongoing management, and any recurrence of ocular redness, cloudiness, or visual impairment should prompt immediate reevaluation (Merck Veterinary Manual, 2025).
