Canine Distemper: Preventable, Serious, and Often Multisystemic
Distemper rarely announces itself as a simple one-system illness; it often touches respiratory, gastrointestinal, and neurologic function.

Distemper is a powerful reminder that vaccines are not abstract paperwork; they prevent diseases that can damage multiple organ systems and the brain.
Once neurologic signs appear, the conversation often becomes more serious and prognosis may worsen significantly.

How Dangerous Is Canine Distemper?
Canine distemper virus (CDV) is a paramyxovirus closely related to the human measles virus. It is one of the most serious infectious diseases of dogs -- before widespread vaccination, it was the leading cause of infectious disease death in dogs worldwide. CDV attacks the respiratory, gastrointestinal, and neurological systems sequentially, and survivors frequently have permanent neurological damage. There is no antiviral treatment; intensive supportive care and early vaccination are the only tools. The distemper component of the core DHPP vaccine provides reliable protection, and distemper in vaccinated dogs is genuinely rare. In unvaccinated populations and shelters with incomplete vaccination coverage, distemper outbreaks still occur.
Transmission and At-Risk Populations
CDV spreads primarily through respiratory secretions -- direct contact with nasal discharge, saliva, or airborne droplets from infected animals. It can also spread through urine and feces. The virus survives poorly in the environment (hours to days at room temperature) compared to parvovirus, but direct animal-to-animal transmission is efficient. At-risk animals: unvaccinated puppies (highest mortality), incompletely vaccinated adults, shelter dogs with unknown vaccination history, and immunocompromised dogs. Wild animals -- raccoons, ferrets, skunks, wolves, foxes -- are susceptible to CDV and can serve as reservoirs.
Clinical Signs: Three Phases
Respiratory phase: Initial signs appear 3-6 days after exposure -- fever (103-104 F), lethargy, loss of appetite, purulent (thick yellow-green) nasal and ocular discharge, coughing, sneezing. The nasal and ocular discharge evolving from clear to mucopurulent is characteristic of distemper vs. other respiratory infections.
Gastrointestinal phase: Vomiting and diarrhea develop in many affected dogs. Weight loss is pronounced. Secondary bacterial pneumonia from bacterial superinfection of the compromised respiratory tract frequently occurs.
Neurological phase: This is what distinguishes distemper from other respiratory illnesses and represents the most devastating aspect of infection. CDV invades nervous system tissue, causing demyelination. Neurological signs develop in approximately 50% of dogs that survive the respiratory phase and can appear weeks to months after the initial illness. Signs: muscle twitches or myoclonus (particularly of the jaw or a limb -- the characteristic "chewing gum fits"), circling, head tilt, ataxia, seizures, blindness, paresis. Neurological signs often progress and are frequently irreversible even if the dog survives.
"Hard pad disease" -- hyperkeratosis (thickening) of the nose and paw pads -- is a specific manifestation of CDV that gave the disease one of its historical names. While not always present, it is nearly pathognomonic when observed.
Diagnosis
CDV diagnosis combines clinical signs, history (unvaccinated puppy with typical signs), and confirmatory testing. Serology (paired acute and convalescent titers) and RT-PCR from conjunctival swabs, nasal swabs, buffy coat, or CSF provide definitive diagnosis. Inclusion bodies (intracytoplasmic and intranuclear) on blood smear examination are a classic finding but not reliably present. Chest radiographs typically show interstitial pneumonia. In-house rapid tests exist but vary in sensitivity compared to PCR.
Treatment
No antiviral treatment exists for CDV. Supportive care includes: IV fluids for dehydration; antibiotics for secondary bacterial pneumonia (amoxicillin-clavulanate, doxycycline); bronchodilators and nebulization for respiratory disease; anticonvulsants (phenobarbital, diazepam) for seizures; vitamin A (some evidence for immune modulation); nutritional support. Intensive nursing care in an isolation unit prevents hospital spread.
Prognosis depends on the severity of neurological involvement at presentation. Dogs with mild or no neurological signs who survive the respiratory phase have reasonable survival prospects. Dogs with severe progressive neurological signs -- ataxia, uncontrolled seizures, progressive demyelination -- have a guarded to grave prognosis. Many clinicians and owners elect euthanasia when neurological signs are severe and progressive.
Vaccination
The CDV component of the DHPP vaccine produces lasting protection. Vaccination protocol mirrors parvovirus: first vaccine at 6-8 weeks, boosters every 3-4 weeks until 16+ weeks. Duration of immunity for distemper is at least 7 years (some studies suggest lifelong). Maternal antibody interference is the main reason for the vaccination series rather than a single early vaccine. AAHA 2022 guidelines recommend the 3-year revaccination interval for the core DHPP vaccine in adult dogs with prior complete vaccination history.
