'Affenpinscher' means 'monkey terrier' in German — and the name fits. Their expressive, almost human-like face and mischievous personality make them one of the most entertaining toy breeds. They're natural comedians who take themselves very seriously, which only makes them funnier.
Affenpinschers are fearless to the point of recklessness. They don't know they weigh 8 pounds and will challenge dogs 10 times their size. Supervise interactions with larger dogs. Their confidence is charming but can get them into trouble if not managed.
A portosystemic shunt (PSS) is an abnormal blood vessel that allows blood from the intestines to bypass the liver and flow directly into systemic circulation. The liver normally removes ammonia, toxins, and metabolic byproducts from portal blood -- when blood bypasses this filtration, these substances accumulate in the brain, causing neurological signs (hepatic encephalopathy). PSS is a congenital defect in most dogs and is particularly common in Yorkshire Terriers, Maltese, Miniature Schnauzers, and Irish Wolfhounds.

Congenital vs. acquired shunts
Congenital extrahepatic shunts (outside the liver): Most common in small and toy breeds. The abnormal vessel is usually single and surgically correctable. These are present from birth but often not diagnosed until 6-18 months of age when owners notice abnormal behavior or poor growth.
Congenital intrahepatic shunts (inside the liver): More common in large breeds, particularly Irish Wolfhounds, Golden Retrievers, and Labrador Retrievers. The shunting vessel runs within the liver parenchyma, making surgery more technically demanding. Often the patent ductus venosus -- a fetal vessel that fails to close after birth.
Acquired shunts: Multiple small vessels develop secondary to portal hypertension from chronic liver disease. Cannot be surgically corrected; management focuses on the underlying liver disease.
Recognizing PSS in a young dog
Classic PSS presentation: a small dog (often a Yorkshire Terrier or Maltese) that is smaller than littermates, grows slowly, seems "off" after meals (post-prandial neurological signs), urinates frequently or has ammonium biurate bladder stones (a unique crystal type caused by elevated urinary ammonia), and has periodic episodes of disorientation, head pressing, circling, aimless wandering, or seizures.
Neurological signs that worsen 2-4 hours after eating are a hallmark -- ammonia from protein digestion peaks post-meal. Some owners report that their dog seems drunk or "not right" after meals and recovers over several hours. These episodes reflect hepatic encephalopathy from ammonia accumulation.
Diagnosis
Bile acid stimulation test: fasting and 2-hour post-prandial bile acid levels are the most sensitive screening test for portosystemic shunting. Values above 25-50 umol/L (depending on lab reference ranges) strongly suggest PSS. Fasting ammonia measurement supports diagnosis. Ultrasound by an experienced sonographer can identify the shunting vessel in many cases. CT angiography provides detailed surgical planning information and identifies the shunt location and size before surgery.
Treatment options
Surgical correction: The treatment of choice for congenital single shunts. Options include ameroid constrictor placement (a ring that slowly occludes the vessel over 4-8 weeks) or cellophane banding (gradual occlusion via inflammation). Sudden complete ligation is no longer the standard -- gradual occlusion allows portal pressure to equilibrate and reduces post-operative portal hypertension crisis risk. Surgical success rates for extrahepatic shunts: 85-95% return to normal liver function. Intrahepatic shunt surgery has higher complication rates but remains the best option for affected large breeds.
Medical management (for non-surgical candidates or pre-surgical stabilization): low-protein diet (prescription hepatic diet), lactulose (reduces intestinal ammonia production by acidifying colon contents and trapping ammonia as non-absorbable ammonium ion), and antibiotics targeting ammonia-producing gut bacteria (metronidazole, neomycin) during encephalopathic episodes. Medical management is not curative -- it controls signs but does not fix the underlying shunt.
Prognosis
Dogs with surgically corrected extrahepatic shunts have an excellent prognosis -- most live normal lifespans. Without surgery, medically managed dogs have significantly shortened lifespans and ongoing quality-of-life challenges. Early diagnosis and referral to a veterinary internal medicine specialist or surgeon with PSS experience is strongly recommended.
