MVD Progression in Cavaliers

Stage A High risk but no disease. Annual cardiac exams. Baseline echocardiogram at age 1–2. Stage B1 Mitral valve leak present but heart not enlarged. No medication needed. Monitor every 6–12 months. Stage B2 Heart enlargement begins. START medication (pimobendan). This is the critical intervention window. Stage C Congestive heart failure symptoms: coughing, breathing difficulty, exercise intolerance. Add diuretics, ACE inhibitors.

Mitral Valve Disease (MVD) affects over 90% of Cavaliers by age 10. The EPIC study proved that starting pimobendan at Stage B2 (heart enlargement, before symptoms) extends symptom-free time by 15 months. Early detection through annual echocardiograms is the single most important thing you can do for your Cavalier.

Syringomyelia (SM) is another devastating condition in Cavaliers — a malformation where the skull is too small for the brain, causing fluid-filled cavities in the spinal cord. Signs include scratching at the neck/air (phantom scratching), sensitivity around the head, and pain. MRI is required for diagnosis.


The Cavalier King Charles Spaniel is a gentle, affectionate toy breed famous for its silky coat and expressive eyes -- and notorious for an extraordinarily high rate of mitral valve disease (MVD). By age 10, nearly all Cavaliers develop heart murmurs. Understanding this breed means understanding its heart.

A Breed Built on Royalty -- and Riddled with Heart Risk

Cavaliers descend from toy spaniels depicted in 16th-century royal portraits. The modern breed was reconstructed in the 1920s to match those paintings, resulting in a flat-faced, large-eyed companion dog that became enormously popular worldwide. The AKC recognized the breed in 1995.

Males average 12-18 lbs; females 10-16 lbs. Lifespan is 9-14 years, though MVD significantly influences longevity. The four recognized colors are Blenheim (chestnut and white), tricolor, black and tan, and ruby.

Mitral Valve Disease: The Cavalier's Defining Health Challenge

MVD affects the mitral valve separating the left atrium and ventricle. In Cavaliers, the valve leaflets thicken and degenerate -- a condition called myxomatous mitral valve disease (MMVD). Blood leaks backward (regurgitation), forcing the heart to work harder until it eventually fails.

Prevalence is staggering: 50% of Cavaliers have heart murmurs by age 5; virtually 100% by age 10. The CAVALIER Health organization and the Cavalier Health website track breeding protocol compliance to reduce early-onset disease.

The MVD Breeding Protocol (developed by cardiologists in 2012) recommends breeding only dogs that are murmur-free at age 2.5 years with both parents murmur-free at age 5. Responsible breeders follow this protocol rigorously.

Stages of Heart Disease

The American College of Veterinary Internal Medicine (ACVIM) classifies MMVD in stages:

  • Stage A: High-risk breed, no murmur yet
  • Stage B1: Murmur present, no cardiac enlargement, no symptoms
  • Stage B2: Murmur present, cardiac enlargement on X-ray or echo, no symptoms
  • Stage C: Current or past signs of heart failure
  • Stage D: End-stage, refractory to standard therapy

The landmark EPIC trial (2016) showed that starting pimobendan (Vetmedin) in Stage B2 dogs delayed onset of heart failure by an average of 15 months. This changed the standard of care -- Stage B2 Cavaliers now routinely receive pimobendan before showing symptoms.

Syringomyelia and Chiari-Like Malformation

The second major heritable condition in Cavaliers involves the skull and spine. Chiari-like malformation (CM) occurs when the brain is too large for the skull, obstructing cerebrospinal fluid flow. This can cause syringomyelia (SM) -- fluid-filled cavities within the spinal cord.

Signs of SM include: scratching at the neck/shoulder area without making contact (phantom scratching), yelping when touched around the head or neck, sensitivity to temperature changes, scoliosis, and in severe cases, weakness or paralysis.

MRI is required for definitive diagnosis. Treatment ranges from pain management (gabapentin, pregabalin, NSAIDs) to surgical decompression. CM/SM affects an estimated 70-95% of Cavaliers to some degree, though many remain asymptomatic.

Eye Conditions

Cavaliers are prone to several eye conditions. Keratoconjunctivitis sicca (dry eye) results from inadequate tear production and requires lifelong cyclosporine or tacrolimus drops. Retinal problems including progressive retinal atrophy (PRA) can cause gradual blindness. Macropalpebral fissure (abnormally large eye opening) predisposes to exposure and injury.

Exercise and Weight Management

Cavaliers adapt well to various living situations but need daily exercise -- typically 30-45 minutes of walking or play. They are not high-energy dogs and do not do well with intense exercise in heat due to their somewhat flat faces (brachycephalic traits are mild but present).

Weight control is critical: even mild obesity accelerates heart disease progression and worsens breathing. Cavaliers are enthusiastic eaters and obesity rates in the breed are high. Feed measured meals, not free-choice.

Grooming Requirements

The silky coat requires brushing 3-4 times per week to prevent mats, especially behind the ears, in the armpits, and on the legs. Many owners opt for professional grooming every 6-8 weeks. Check and clean ears weekly -- the long, floppy ears trap moisture and are prone to infection.

Temperament and Training

Cavaliers rank among the gentlest of all breeds. They are typically excellent with children, other dogs, and cats. They were bred as lap dogs and companions -- separation anxiety is common when left alone for extended periods.

Training is generally straightforward; Cavaliers are eager to please and respond well to positive reinforcement. They excel at therapy dog work due to their calm, empathetic nature.

What to Ask Your Breeder

Always request documentation of: annual cardiac exams (cardiologist-certified, not just vet auscultation), CM/SM MRI screening, eye CAER exam, hip evaluation, and patella OFA results. The Cavalier Health website maintains a public database of health-tested dogs. Red flags include breeders who cannot produce cardiologist exam records or who claim their lines are 'heart-healthy' without documentation.
