The Treatment That Kills the Fish -- and the Tank
More fish die from incorrect treatment than from the diseases being treated. A fish keeper sees a sick fish, panics, adds three different medications simultaneously, and kills both the disease and the beneficial bacteria colony that processes ammonia in the tank. Ammonia spikes. More fish die. The solution looks like it caused the problem -- and in this case, it did.
Accurate diagnosis comes first. The most common misdiagnosis: treating what looks like ich (white spots) with the wrong treatment for the actual pathogen present, or treating a bacterial infection with antifungals. A 10x loupe magnifier or microscope scraping is the gold standard for parasitic identification -- for most home aquarists, treatment based on careful visual diagnosis of the distribution, size, and appearance of lesions is the practical alternative. Know what you're treating before you add anything to the water.
Hospital Tanks: The Non-Negotiable Setup
A hospital tank (also called a quarantine tank) is a separate, bare-bottom aquarium used for treating sick fish away from the main display. Benefits: you can use medications that would destroy beneficial bacteria in the main tank; you can dose accurately without calculating for live plants, carbon, or decorations that absorb medications; you can observe the sick fish closely; and you prevent spread to healthy fish. Minimum setup: 10-20 gallons, a sponge filter pre-seeded from the main tank, and a heater. No substrate, no decorations.
A quarantine tank also serves as the mandatory 4-week arrival period for all new fish -- this single practice prevents most disease introductions. New fish look healthy at the store and carry ich, velvet, internal parasites, and bacterial infections that appear only after the stress of transport. A 4-week quarantine allows disease to manifest in isolation rather than spreading to an established community tank.
Ich: The White Spot That Almost Every Aquarist Will Face
Ichthyophthirius multifiliis (ich) is ubiquitous in freshwater fishkeeping -- virtually every fish has been exposed, and stress triggers the disease when immunity is compromised. Ich appears as small white salt-grain spots, most visible on the fins. The life cycle has three stages: tomonts (cysts on the tank bottom, immune to treatment), theronts (free-swimming, vulnerable to treatment), and trophonts (on the fish, partially protected by the fish's mucus). Effective treatment targets the free-swimming theront stage.
Standard treatment for freshwater ich: raise temperature to 82-86 degrees F (accelerates the life cycle, speeds up the vulnerable stage), combined with either malachite green/formalin products or copper sulfate in a hospital tank. The treatment period is 2 weeks minimum -- the tomont on the substrate can persist for 7-10 days at higher temperatures before hatching theronts. Stopping treatment early because spots disappeared leaves tomonts intact to restart the cycle.
Antibiotics: When to Use Them and When Not To
Bacterial infections in fish present as: hemorrhaging (red streaks in fins, skin lesions), fin rot (edges of fins deteriorating), columnaris (cottony white patches with a saddle-shaped distribution across the back), dropsy (pinecone scale appearance from fluid accumulation), and pop-eye (exophthalmia, one or both eyes protruding). These require antibiotic treatment; the choice depends on whether the infection is gram-positive or gram-negative.
Kanamycin, erythromycin, and tetracycline target gram-positive bacteria. Trimethoprim-sulfamethoxazole, florfenicol, and metronidazole address gram-negative and anaerobic bacteria. Never add antibiotics to the main tank -- they eliminate beneficial nitrifying bacteria, cause ammonia spikes, and contribute to environmental antibiotic resistance. Treat in the hospital tank, complete the full course, and follow with biological filter restoration using a bacterial supplement before returning fish to the main tank.
