The Latency Cycle

After initial infection -- typically in kittens from an infected mother -- FHV-1 travels up sensory nerves and establishes permanent latency in the trigeminal ganglion. The virus becomes dormant, invisible to the immune system, and undetectable on PCR. Stress triggers reactivation: the virus travels back down nerve pathways and replicates in epithelial tissue, causing recurrent symptoms. Classic triggers: new pets, moving, veterinary visits, boarding, surgery, other illness, seasonal changes. Approximately 45% of cats shed virus after experimental corticosteroid administration (a surrogate for physiological stress).

Recognizing a Flare

Respiratory flares: nasal discharge (starts clear, may become yellow-green with secondary bacterial infection), sneezing fits, congestion causing mouth breathing, fever. Ocular flares: conjunctivitis (red, swollen conjunctiva), ocular discharge, corneal ulceration (dendritic ulcers visible with fluorescein staining at a vet). Corneal ulcers are painful and require prompt treatment -- untreated, they can progress to corneal sequestrum or even perforation. Skin flares are less common: facial ulceration at the nostrils or mouth corners.

Treatment During Flares

Ocular flares: topical idoxuridine 0.1%, trifluridine 1%, or cidofovir 0.5% eye drops applied every 6-8 hours for 7-10 days significantly reduce corneal ulcer healing time. Cidofovir has the advantage of twice-daily dosing. Oral famciclovir (90 mg/kg every 8 hours -- this is cat-specific dosing, not human doses) reduces viral replication systemically and is the preferred oral antiviral for severe cases. Do not use acyclovir in cats -- poor oral bioavailability and potential nephrotoxicity. Secondary bacterial infections require antibiotics (doxycycline 5 mg/kg once daily or azithromycin 5-10 mg/kg once daily).

L-Lysine: The Controversy

L-lysine supplementation (250-500 mg twice daily) has been used for decades based on the hypothesis that lysine competes with arginine, which FHV-1 requires for replication. However, a 2015 systematic review in BMC Veterinary Research found no convincing evidence of benefit, and some studies found lysine supplementation increased shedding in some cats. The AAFP 2020 guidelines removed lysine from recommended management. The supplement appears low-risk but evidence for efficacy is poor. Some veterinarians continue recommending it; discuss with your vet.

Reducing Flare Frequency

Stress minimization is the most evidence-based intervention: Feliway diffusers (synthetic feline facial pheromone) reduce behavioral stress markers. Environmental enrichment, consistent routine, and slow introductions for new pets help. For cats with severe recurrent disease, daily prophylactic famciclovir has been used in some cases but evidence is limited. Ensure the cat is up to date on FHV-1 vaccination -- while vaccines do not eliminate infection, they reduce severity and viral shedding during flares.
