Urethral obstruction (urinary blockage) in male cats is one of the most time-critical emergencies in small animal medicine. The urethra in male cats is anatomically narrow (approximately 1.5 mm diameter) compared to females, making males uniquely vulnerable to obstruction from urethral plugs (mucus, crystals, cells), uroliths (stones), or urethral spasm from inflammation. A cat that cannot urinate develops life-threatening hyperkalemia (elevated blood potassium causing cardiac arrhythmias) and uremia within 24 to 48 hours. Fatality rates in untreated cases approach 100% within 3 to 4 days.

Recognizing a Blocked Cat

The early sign of urethral obstruction is frequent attempts to urinate with little or no output: the cat visits the litter box repeatedly, strains in a squatting position, and produces drops or nothing. Many owners mistake this for constipation or diarrhea. Within 12 to 24 hours, the cat becomes painful (crying, hunched posture), lethargic, stops eating, and may vomit. The bladder becomes tense and palpable -- distended like a firm water balloon in the caudal abdomen. At 24+ hours: profound weakness, collapse, hypothermia, and cardiac arrhythmias from hyperkalemia develop.

Any male cat that visits the litter box multiple times within 30 minutes without producing a normal urine stream should be examined immediately. Do not wait to see if he improves. A cat urinating even a small amount is not definitively unblocked -- a partial obstruction may allow drops to pass while the bladder continues to distend.

Emergency Treatment

Diagnosis at the clinic: physical exam detects the distended bladder; urethral catheter confirms obstruction when it cannot pass. Bloodwork (BMP/chemistry) reveals hyperkalemia (normal cat: 3.5 to 5.5 mEq/L; blocked cats often present at 7 to 9+ mEq/L), elevated BUN and creatinine from uremia, and metabolic acidosis. ECG may show bradycardia and spiked T waves from hyperkalemia.

Treatment: IV catheter placement, IV fluids (0.9% NaCl for hyperkalemia correction), treatment of severe hyperkalemia if ECG changes are present (calcium gluconate 50 to 100 mg/kg IV to stabilize cardiac membrane, insulin + dextrose to shift potassium intracellularly), urethral catheterization under sedation or anesthesia to relieve obstruction and flush the bladder. Urinary catheter remains in place 24 to 72 hours for decompression and monitoring. Median hospitalization: 2 to 3 days. Total cost at most emergency practices: $1,500 to $4,000 depending on severity and length of hospitalization.

Prevention

Risk factors for urethral obstruction: male sex, dry food only diet (increases urine concentration and promotes crystal formation), low water intake, obesity, and indoor sedentary lifestyle. Prevention strategies: wet food as the primary or exclusive diet (dramatically increases water intake), water fountains to encourage drinking, weight management, multi-cat litter box availability (3+ boxes for 2 cats) to reduce urination-associated stress. Cats with recurrent obstructions are candidates for perineal urethrostomy (PU surgery), which widens the urethra surgically; PU eliminates the anatomical narrowing that predisposes to blockage but does not prevent underlying urinary tract disease.
