Hydrotherapy sounds improbable if your mental image of feline medicine is a cat fleeing the bathroom at the sight of a faucet. Yet physical rehabilitation specialists increasingly use water-based therapy as one tool inside a broader plan for cats with pain, weakness, neurologic deficits, or recovery needs after injury or surgery. The key idea is not “make the cat swim.” It is to use buoyancy, resistance, temperature, and carefully controlled movement in a way the individual cat can tolerate and benefit from (Goldberg, 2025; AAFP, 2024).

The recent feline rehabilitation review by Goldberg describes hydrotherapy as one of many modalities that can be part of restoring movement and function after injury, illness, or trauma, and notes that these methods may be used in both acute and chronic conditions (Goldberg, 2025).

That matters because many owners hear the word and picture forced swimming. In reality, the goal is controlled therapeutic movement, not panic. Water can make stepping easier by offsetting some body weight, while also adding gentle resistance that land exercises cannot provide the same way. For the right cat, that combination can support rebuilding after orthopedic surgery, help a weak neurologic patient practice coordinated movement, or let an arthritic cat work without the same level of joint loading felt on dry ground (Goldberg, 2025; AAHA/AAFP, 2022).

Which cats are the best candidates for hydrotherapy?

The short answer is not “all cats with sore joints.” The better answer is cats whose diagnosis, pain control, temperament, and physical stability make water-assisted exercise likely to help more than it stresses them. Goldberg’s review emphasizes that rehabilitation can be useful for acute and chronic conditions and that identifying when a cat is painful, then prioritizing therapies accordingly, is crucial. In practice, candidates may include some cats recovering from injury or surgery, some chronic pain patients, and some neurologic patients needing carefully graded exercise (Goldberg, 2025).

Cats with degenerative joint disease are a common example because chronic pain frequently reduces mobility and quality of life. The AAFP chronic pain toolkit notes that painful degenerative joint disease is common in cats and is associated with decreased activity and function. A rehab veterinarian may decide that an underwater treadmill or other supported exercise is worth trying when a cat has enough emotional tolerance for handling and enough medical stability to work safely, particularly when land exercise alone is difficult (AAFP, 2024).

Why can water-based exercise help movement and pain?

Water changes physics in a way that can be therapeutically useful. There are also medical reasons to say no or not yet. Unstable heart or respiratory disease, uncontrolled pain, certain wounds, untreated infection, or severe weakness can make hydrotherapy inappropriate until the cat is better stabilized. Multimodal pain guidelines emphasize tailoring rehabilitation and exercise to the individual patient, not forcing a technique because it is fashionable. The best rehab teams are willing to abandon water completely when the cat’s temperament or medical status says that dry-land therapy or environmental modification is the kinder and safer route (AAHA/AAFP, 2022; Goldberg, 2025).

The point is measured progress, not proving that the cat can tolerate a long workout. What else should owners do at home if a cat is in rehab?

Owners should also track function with the same seriousness they give medication schedules. Rehab is successful when the cat can do more with less discomfort in ordinary life, not only when it performs well in the clinic. If hydrotherapy leaves the cat stressed, exhausted, or less willing to move later, that is useful information and should be reported promptly so the plan can be changed (FelineVMA, 2024; AAFP, 2024).

When should you pause hydrotherapy and call the veterinary team?

Pause the plan and call if your cat shows increased pain, panting, heavy stress, new lameness, weakness after sessions, wound problems, or reluctance to move compared with baseline. Rehabilitation should improve function, not produce a stress hangover that lasts the rest of the day. Because cats hide discomfort, even subtler post-session changes such as hiding, reduced appetite, or a hunched posture deserve a report to the rehab team (FelineVMA, 2024; Goldberg, 2025).

Seek more urgent care if your cat develops breathing difficulty, collapse, severe lethargy, or sudden neurologic worsening. Those are not normal exercise responses.
