'Manx syndrome' is an umbrella term, not a single neat diagnosis. Incontinence, constipation, hind-limb weakness, and abnormal gait all warrant attention in these lines, especially in very short-tailed kittens.
The Manx is a naturally tailless cat from the Isle of Man, caused by a dominant mutation in the T-box transcription factor gene. The same mutation that removes the tail also affects spinal development -- homozygous Manx kittens die in utero, and even heterozygous individuals have elevated risk of Manx Syndrome, a collection of spinal defects causing bowel, bladder, and motor dysfunction. The Cymric is a longhaired Manx variant with identical genetics. Both breeds require educated owners who monitor for early neurological signs.
The Taillessness Spectrum
Not all Manx cats are fully tailless. The degree of tail reduction varies: rumpy (no tail, flat rump), rumpy-riser (small rise of bone), stumpy (short tail remnant of 1--4 vertebrae), and longy (tail approaching normal length). The genetic mutation affects variable portions of the caudal spine; the same cat can produce kittens across the entire spectrum. Show-standard Manx are rumpies; all degrees occur in breeding populations.
Manx Syndrome
The Manx mutation can affect development of the entire posterior spinal column. Manx Syndrome encompasses: spina bifida (incomplete vertebral arch closure), sacrococcygeal agenesis (absence of sacral vertebrae), and failure of normal gut and bladder innervation. Affected cats show: fecal and urinary incontinence, hind limb weakness or paralysis, bunny-hopping gait, and spinal pain. Signs typically appear by 4 months of age. Severity ranges from mild incontinence manageable with dietary management to severe paralysis requiring euthanasia. Estimated 20% of Manx cats have some degree of Manx Syndrome.
Buying Responsibly
Purchase Manx or Cymric kittens only from breeders who allow assessment at 4--5 months of age, after neurological signs would have become apparent. A kitten that is pain-free, has normal gait, and shows no incontinence at 5 months is very unlikely to develop severe Manx Syndrome later. Reputable breeders do not place kittens at 8 weeks before potential neurological signs are visible. Ask specifically about Manx Syndrome and whether the kitten has been evaluated by a veterinarian.
Management of Mild Manx Syndrome
Cats with mild incontinence from Manx Syndrome can have excellent quality of life with appropriate management: regular manual bladder expression if incomplete emptying occurs, high-fiber diet for bowel regularity, and monitoring for urinary tract infections (a consequence of bladder dysfunction). Regular veterinary follow-up every 6 months allows early identification of progression.
