Antifreeze tastes sweet to dogs and cats, and as little as a few teaspoons of the ethylene glycol kind can be lethal to a mid-size dog, let alone a cat. The sweetness that draws animals to it is exactly what makes it so dangerous

Ethylene glycol, the main ingredient in most conventional antifreeze, tastes sweet, which is part of why dogs and cats will lap up a puddle in a driveway or garage without hesitation. It takes very little: ingestion of roughly 4.4 mL per kilogram of body weight can be lethal in dogs, and cats are even more sensitive, needing only around 1.5 mL per kilogram.

For a 10-pound cat, that's a couple of teaspoons.

The scariest part is the fake recovery in the middle

The first stage looks like drunkenness: stumbling, vomiting, and acting disoriented within 30 minutes to 12 hours of ingestion.

That's followed by a deceptive quiet period, roughly 12 to 24 hours in, where the animal often seems to improve, even though the kidneys are already being damaged underneath. By 24 to 72 hours, that hidden damage becomes acute kidney failure, and by then, treatment options have narrowed considerably.

The antidote has a clock attached to it

Fomepizole, the antidote used in dogs, works by blocking the enzyme that converts ethylene glycol into its most toxic byproducts, but it only works if given before that conversion has already happened - generally within about 8 to 12 hours of ingestion.

That narrow window is why any known or suspected antifreeze exposure is treated as a true emergency, not a wait-and-see situation, even if the animal looks fine in the moment.

The Merck Veterinary Manual describes ethylene glycol toxicosis as progressing through a distinct staged clinical course: early neurologic signs (ataxia, vomiting, depression) within roughly 30 minutes to 12 hours, an apparent but deceptive improvement in the following 12 to 24 hours, and progressive acute kidney injury from about 24 to 72 hours after ingestion. The manual lists lethal dose estimates of roughly 4.4 mL/kg in dogs and roughly 1.5 mL/kg in cats, and identifies fomepizole (or ethanol, where fomepizole is unavailable) as most effective when administered within about 8-12 hours of ingestion, before the compound is metabolized into its more toxic byproducts.
