Adrenal gland disease is so common in ferrets that it affects an estimated 70-80% of pet ferrets in the US by age 5-6. It's not a fringe medical problem -- it's essentially the default health trajectory for an American ferret. The disease causes the adrenal gland to overproduce sex hormones, leading to a distinctive clinical picture: hair loss beginning at the tail base, itching, and in females, a swollen vulva that can be mistaken for estrus even in spayed animals.

Why American Ferrets Are So Susceptible
A leading hypothesis involves early spay/neuter practice. The vast majority of US ferrets are spayed or neutered at 4-6 weeks old by commercial breeders, far earlier than the equivalent age in European ferrets (typically 6-12 months or later). Early removal of gonadal feedback may cause the pituitary gland to chronically overstimulate the adrenal gland's sex hormone-producing cells, leading to hyperplasia or adenoma formation over years. European ferrets, typically altered later or not at all, have dramatically lower adrenal disease rates -- lending significant support to this hypothesis.
Clinical Signs
The most recognizable sign: symmetric, progressive alopecia (hair loss) starting at the tail and advancing up the back and sides. Unlike a nutritional deficiency (which typically affects the coat quality throughout), adrenal-related hair loss is distinctly patterned. The skin in affected areas may be mildly thickened or slightly pink.
In spayed females: vulvar enlargement resembling estrus (because the excess hormones from the adrenal gland mimic ovarian hormones). Untreated, this can lead to aplastic anemia -- the excess estrogen suppresses bone marrow, causing life-threatening anemia. In males: prostatic enlargement causing straining to urinate, sometimes complete urinary obstruction (a medical emergency).
Other signs: itching (pruritus), sometimes muscle wasting, lethargy, and weight loss in advanced cases.
Diagnosis
Diagnosis is based primarily on clinical presentation in a ferret over 2 years old -- the combination of patterned hair loss, vulvar enlargement, or urinary straining in an older ferret is highly characteristic. Adrenal ultrasound can confirm adrenal enlargement or asymmetry. Hormone panels (measuring estradiol, androstenedione, 17-hydroxyprogesterone) are available through the University of Tennessee College of Veterinary Medicine and can confirm adrenal hormone excess.
Treatment Options
Surgical adrenalectomy: removal of the affected gland (or both in bilateral disease). This is the only true cure. However, disease in the remaining gland often develops later, and surgery carries risk in older or debilitated ferrets. A ferret veterinarian with ferret surgical experience is essential.
Medical management: Deslorelin acetate (Suprelorin implants, available compounded in the US) is a GnRH agonist that desensitizes the pituitary and dramatically reduces adrenal stimulation. Implants last 8-16 months and require replacement. They don't remove the diseased gland but manage symptoms effectively in most ferrets. This has become the primary treatment approach for many ferrets that are not surgical candidates or whose owners prefer medical management.
Prognosis
Adrenal disease is manageable but progressive. Most ferrets with appropriate treatment live comfortable lives for years after diagnosis. The main concern is the downstream effects: aplastic anemia in spayed females and urinary obstruction in males can be life-threatening if not caught early. Annual to semi-annual wellness exams beginning at age 3, including vulvar inspection in females and urinalysis in males, catch complications before crisis.
