Urinary calculi (bladder stones) are among the most common surgical conditions in guinea pigs, particularly affecting females over 2-3 years of age, though males are not immune. Calcium oxalate and calcium carbonate stones are the most common types. A guinea pig with bladder stones shows signs that are easy to confuse with urinary tract infection or behavioral changes -- blood in the urine, hunched posture, teeth grinding, or vocalization during urination. Diagnosis requires radiographs. Medical dissolution does not work for the stone types common in guinea pigs; surgery is the definitive treatment for stones large enough to cause obstruction or significant pain.

Why Guinea Pigs Form Bladder Stones
Guinea pigs' urinary systems handle calcium differently than most mammals. They absorb a high proportion of dietary calcium from the intestine regardless of the body's actual calcium needs (rabbits have a similar physiology). Excess calcium is excreted in the urine, making guinea pig urine normally cloudy and white -- this is normal calcium carbonate precipitation, not a pathological finding. However, in some individuals, conditions favor the precipitation and aggregation of calcium salts into solid stones rather than remaining in suspension: low water intake (concentrated urine), high-calcium diet, urinary tract infection (biofilm from bacteria provides a nucleus for crystal aggregation), and possibly genetic predisposition.
Dietary factors: excess calcium in the diet accelerates stone formation. High-calcium vegetables -- spinach, kale, collard greens, beet greens -- should be limited rather than fed as the primary vegetable component. Excellent lower-calcium alternatives: romaine lettuce, cucumber, bell peppers, zucchini, and fresh herbs. This does not mean eliminating all calcium-containing foods; guinea pigs need calcium. It means avoiding serving high-calcium greens daily as the primary vegetable.
Recognizing Bladder Stones
Clinical signs range from subtle to severe depending on stone size and location:
Mild/early signs: Hematuria (blood in urine -- the urine may appear pink, red, or orange on the cage floor or on white bedding; note that normal guinea pig urine can range from yellow to orange-red depending on diet, so true blood may be hard to distinguish without a vet evaluation); more frequent urination attempts; occasional teeth grinding or hunched posture during urination.
Significant signs: Straining in the litter area with reduced or absent urine output; vocalization or grinding of teeth during urination attempts; weight loss; reduced food intake; reluctance to move. In males, a stone lodged in the urethra causes complete obstruction (emergency).
Obstructive emergency (primarily males): No urine produced for 12+ hours; visibly painful; bloated or hard lower abdomen. Complete urethral obstruction is rapidly fatal in guinea pigs. The male urethra is narrow enough that even a 2-3 mm stone can cause obstruction. Emergency deobstruction and surgery required.
Diagnosis
Radiographs (X-rays) are the standard diagnostic tool -- calcium-containing bladder stones are radiopaque (clearly visible on X-ray). Most guinea pig stones are easily identified on plain lateral and ventrodorsal radiographs. Ultrasound supplements radiographs for evaluating the bladder wall and any concurrent kidney involvement. Urinalysis evaluates for concurrent urinary tract infection (white blood cells, bacteria) that often accompanies or causes stones.
Treatment
Medical dissolution: does not work reliably for calcium oxalate or calcium carbonate stones (the types common in guinea pigs). Cranberry juice and urinary acidifiers are not effective for calcium stone dissolution. Antibiotics are used to treat concurrent UTI but do not dissolve the stone. Increasing water intake via wet food reduces stone recurrence risk post-surgery but does not dissolve existing stones.
Surgical cystotomy: the definitive treatment. An exotic mammal-experienced veterinarian performs a procedure under general anesthesia to open the bladder, remove the stone(s), and flush the urinary tract. Success rates in otherwise healthy guinea pigs are good; the primary risks are anesthesia (guinea pigs are higher anesthetic risk than dogs or cats due to their sensitivity to respiratory depression), and post-operative ileus (GI shutdown from pain and anesthesia -- critical to monitor and treat with supportive care and pain management post-operatively). Recovery: 1-2 weeks. Recurrence rate is significant -- 30-50% of guinea pigs form new stones within 1-2 years, making dietary modification post-surgery important.
