Guinea Pig Health at a Glance
Respiratory infections are among the most common and most rapidly fatal diseases in guinea pigs. Unlike dogs and cats that may show illness gradually, guinea pigs can go from appearing slightly off to critical within 24-48 hours. The most dangerous respiratory pathogens in guinea pigs include Bordetella bronchiseptica (bacteria transmitted from rabbits and dogs), Streptococcus pneumoniae, and Sendai virus. A guinea pig showing any combination of labored breathing, nasal discharge, lethargy, or reduced food intake needs veterinary evaluation within 24 hours -- not monitoring at home for 3 days.

Bordetella bronchiseptica: the most dangerous common pathogen
B. bronchiseptica causes kennel cough in dogs and respiratory disease in rabbits -- and it kills guinea pigs with high efficiency. Guinea pigs are extremely susceptible; exposure from a rabbit or dog with active infection can cause fatal pneumonia within days. This is why housing guinea pigs with rabbits is strongly contraindicated by exotic veterinarians -- even an asymptomatic rabbit can shed B. bronchiseptica at levels lethal to a guinea pig in the same cage. If you have dogs with recent respiratory illness, consider isolation from guinea pigs until the dog has been treated and cleared.
Recognizing respiratory distress: what to watch for
Early signs (see a vet today): nasal discharge (clear or colored), sneezing more than occasionally, reduced appetite, sitting with a hunched posture. Emergency signs (emergency vet now): labored breathing with visible rib expansion and visible effort, open-mouth breathing (guinea pigs are obligate nasal breathers -- open-mouth breathing is a crisis), blue or pale gums indicating hypoxia, complete anorexia, inability to stand. Guinea pigs have extremely high respiratory rates (80-130 breaths per minute at rest) -- any marked increase or visible effort in breathing is immediately alarming.
Diagnosis and treatment requires exotic animal veterinary experience
Diagnosis: physical exam + thoracic auscultation + radiographs (ideally two views -- lateral and dorsoventral) + possible culture and sensitivity of nasal discharge. Treatment depends on severity and causative organism. Antibiotics: trimethoprim-sulfa (Bactrim) is often first-line; enrofloxacin (Baytril) for gram-negative organisms; doxycycline for Bordetella. Nebulization therapy (antibiotic and saline mist) supplements oral antibiotics in moderate cases. Critical patients need oxygen supplementation, assisted feeding, and hospitalization. Treatment must begin before culture results in severe cases.
Environmental factors that increase infection risk
Dusty bedding (fine sawdust, cedar shavings) irritates respiratory mucosa and increases susceptibility to secondary infection. Use dust-free paper bedding (Carefresh, Oxbow Pure Comfort) or kiln-dried pine. Poor ventilation concentrates ammonia from urine, which directly damages respiratory epithelium -- clean litter areas daily minimum, full bedding change 1-2 times per week. Temperature extremes cause stress-induced immune suppression. Overcrowding in multiple-guinea-pig housing increases transmission. New guinea pigs should be quarantined for 2-3 weeks before introduction to existing guinea pigs.
Prevention through housing management
Never house guinea pigs with rabbits. Quarantine all new animals. Avoid exposure to dogs with respiratory illness. Purchase animals only from reputable sources (avoid pet stores where animals from multiple sources are commingled). Guinea pigs purchased from high-density environments often carry subclinical respiratory infections that become clinical under the stress of transportation and a new home. A wellness veterinary exam within 72 hours of purchase catches early disease before a guinea pig becomes critically ill.
