Hamsters are prey animals that mask illness until severely compromised. Warning signs requiring immediate veterinary attention: sitting hunched or at the bottom of the cage, wet tail (profuse watery diarrhea -- often fatal within 24-48 hours untreated), labored breathing, eye discharge or sealed shut, sudden weight loss, or bleeding. Hamsters have a resting heart rate of 250-500 beats per minute and body temperature of 99-100 degrees F; their rapid metabolism means disease progresses in hours, not days.

Wet tail is the most feared disease in young hamsters
Wet tail (proliferative ileitis) is caused by Lawsonia intracellularis bacteria and primarily strikes Syrian hamsters 3-8 weeks old, often within days of arriving in a new home. The stress of weaning, shipping, and rehoming suppresses the immune system and allows the bacteria to proliferate. Symptoms: profuse liquid diarrhea soiling the tail and hindquarters, lethargy, hunched posture, and reluctance to move.
Wet tail kills within 24-72 hours without treatment. At the first sign, the hamster needs immediate veterinary care -- antibiotics (typically enrofloxacin or metronidazole), supportive fluids, and warmth. Even with treatment, mortality is high. Prophylactically, avoid purchasing hamsters showing any signs of diarrhea, quarantine new hamsters for 2 weeks, and minimize stress during the adjustment period.
Hibernation vs. death: a critical distinction
Syrian hamsters can enter a torpor-like state (pseudohibernation) when ambient temperature drops below 50 degrees F. In this state, they appear dead: cold to the touch, rigid, minimal breathing. This is not true hibernation -- it is a dangerous stress response. A hamster in torpor can die if not warmed gradually. Warm the hamster slowly by holding it against your body for 30-60 minutes, then provide food and water. Prevent recurrence by keeping the room above 60 degrees F.
Dwarf hamsters (Campbell's, Winter White, Roborovski) do not pseudohibernate as readily as Syrians. Any hamster found apparently dead should be warmed and observed for 30 minutes before assuming death.
Diabetes in dwarf hamsters
Campbell's dwarf hamsters have a genetic predisposition to type 2 diabetes -- prevalence in some breeding lines exceeds 70%. Signs: increased water intake (polydipsia), increased urination (polyuria), weight loss despite good appetite, and cataracts. Diagnosis is confirmed by blood glucose measurement (normal: 60-130 mg/dL; diabetic: consistently above 200 mg/dL).
Management focuses on diet: eliminate sweet treats, fruit, and high-carbohydrate foods. A diet based on hamster pellets and low-sugar vegetables controls progression. Insulin is rarely used in hamsters due to dosing difficulty and stress of injections. Well-managed diabetic hamsters can live relatively normal lives with careful dietary control.
Respiratory infections and their prevention
Upper respiratory infections present as nasal discharge (clear or cloudy), sneezing, labored breathing, and lethargy. Bacterial causes (Streptococcus, Pasteurella) are common; viral causes also occur. Treatment requires antibiotics from a vet experienced with small mammals. Prevention: keep the cage clean (ammonia buildup from soiled bedding damages respiratory mucosa), avoid drafts, and quarantine new animals.
Cedar and pine bedding emit aromatic oils that cause chronic respiratory irritation. Use paper-based bedding only. Scented bedding products, air fresheners, and cigarette smoke in the room also damage hamster lungs.
Cheek pouch impaction and prolapse
Hamsters carry food in cheek pouches that can extend to their shoulders. Sticky foods (peanut butter, candy, some fresh foods) can adhere to the pouch lining and cause impaction. A swollen cheek that does not deflate over several hours indicates impaction. Prolapse (pouch tissue turning inside out and protruding from the mouth) requires emergency veterinary care -- the tissue must be replaced and the underlying cause addressed within hours to prevent necrosis.
