Stress is the primary trigger. Lawsonia intracellularis bacteria are often already present in low numbers; stress causes overgrowth. Reducing stress is the single most effective prevention.

Wet tail (proliferative ileitis) is a bacterial intestinal infection in hamsters, primarily affecting Syrian hamsters under 12 weeks old, that causes profuse watery diarrhea, rapid dehydration, and death within 24-48 hours if untreated. The name comes from the characteristic wet, matted fur around the hindquarters and tail. This is the most common acute killer of newly purchased hamsters. A hamster with wet tail needs a veterinarian the same day symptoms appear -- waiting overnight is often fatal.

What Causes It and Why Young Hamsters Are Vulnerable

Wet tail is caused primarily by overgrowth of Lawsonia intracellularis, a bacterial pathogen that infects rapidly dividing intestinal cells. Young hamsters (4-12 weeks) are most vulnerable because their intestinal cells are dividing rapidly during growth, providing optimal conditions for Lawsonia replication. The bacteria cause proliferative changes in the ileum (the last section of small intestine), disrupting normal absorptive function and triggering massive fluid loss into the intestinal lumen.

Trigger factors include: stress from transport or rehoming (the most common trigger -- many hamsters develop wet tail within days of being brought home from a pet store or breeder), overcrowding, rapid dietary changes, temperature extremes, and concurrent illness. The stress response elevates cortisol, which suppresses gut immunity and alters intestinal motility, allowing Lawsonia to proliferate unchecked.

Dwarf hamsters (Roborovski, Campbell's, Winter White, Chinese) develop diarrhea less commonly from Lawsonia but can develop similar-appearing diarrheal illness from other bacterial causes, dietary indiscretion, or giardia. Treatment principles are similar.

Recognizing Wet Tail: Symptoms and Progression

Early signs (first 12-24 hours): loose stool, decreased activity, hunched posture, reduced food intake, slight dampness around the tail. The hamster may seem lethargic but still responsive.

Progressed disease (24-48 hours): profuse watery diarrhea staining the entire hindquarters, strong foul odor, severe lethargy, sunken eyes (dehydration), refusal to eat or drink, dragging hindquarters. Some hamsters develop rectal prolapse -- a pink tissue protrusion from the anus -- which requires immediate veterinary reduction.

Terminal stage: unconsciousness, hypothermia (cold to the touch), seizures. At this stage, survival is unlikely even with intensive care. This progression can happen in under 48 hours from first symptoms in severe cases.

Treatment: What the Vet Will Do

Veterinary treatment for wet tail includes:

Antibiotics: Enrofloxacin (Baytril) is most commonly used -- typically 5-10 mg/kg orally twice daily for 5-7 days. Metronidazole is sometimes added for its antiprotozoal and anaerobic bacterial coverage. Doxycycline is an alternative. Antibiotic choice depends on the veterinarian's assessment and what oral formulations are available for small animals.

Fluid therapy: Subcutaneous fluids (saline or Lactated Ringer's solution) to correct dehydration. Severely affected hamsters may receive 1-2 mL subcutaneously every 4-6 hours at the clinic. Owners can be taught to give subcutaneous fluids at home in mild cases.

Anti-diarrheal and gut motility support: Bismuth subsalicylate is sometimes used (avoid aspirin-containing products); probiotic pastes formulated for small animals help restore normal gut flora.

Nutritional support: Critical care formula (e.g., Oxbow Critical Care for Herbivores) syringe-fed to maintain caloric intake if the hamster won't eat voluntarily.

Do not give human anti-diarrheal medications containing loperamide (Imodium) without veterinary guidance -- dosing for a 100-200 gram hamster is extremely difficult to get right and overdose is possible.

Home Care While Awaiting the Vet

Before reaching a vet, keep the hamster warm (a heating pad set to low under half the cage, so the hamster can move off it), offer fresh water and gentle foods (plain cooked chicken, baby food squash without additives), and gently clean the soiled hindquarters with a warm damp cloth to prevent urine scald. Do not attempt to force fluids orally -- a severely ill hamster may aspirate. These measures buy time; they do not cure the infection.

Prevention in New Hamsters

The highest-risk period is the first 2 weeks after bringing a hamster home. Minimize stress: set up the cage completely before the hamster arrives, place the transport container inside the cage and let the hamster exit on its own, avoid handling for the first 2-3 days, keep the environment quiet and at stable temperature (65-75 degrees F), and do not change the diet abruptly. Gradually transition food over 5-7 days if switching brands. Quarantine new hamsters away from existing pets.
